Fall 2008 Registration
Register by mail at address below or at the Prouty Building on the following dates:

Players are NOT allowed to wear jewelry of any kind to practices or games (including newly pierced earrings).  No jeans, jean shorts, tear- aways; carpenter pants or any other pants or shorts with pockets or belts of any kind.  Also, there will be no CASTS or gum allowed.  Athletes must provide shin guards and they must be worn to ALL practices and games.

Emergency medical release waivers MUST be filled out PRIOR to any participation.  (This includes practices and games).  They need to be filled out each calendar year.

PYSL supplies the complete uniform and it is REQUIRED to be worn for your child to play.

BLACK UNIFORM SHORTS ARE A REQUIRED PART OF THE UNIFORM AND PARTICIPANTS WILL NOT PLAY WITHOUT THEM ! ! !

There will be NO REQUESTS  honored other than for a sponsors child  or a coaches child.

There will  be a 50% fee assessed for players that drop out after registration

and NO REFUND after teams are picked.

Players will be added to teams ONLY prior to teams being handed out.

There will be no registrations taken after .

There will be a  $25.00 charge for ALL returned checks.

PLEASE MAKE SURE THAT ALL INFORMATION ON THE FORM IS FILLED OUT AND THAT IT IS CORRECT.  THIS WILL MAKES THINGS RUN MORE EFFICIENTLY.

Fees:  K-8 is  $45.00 each for the first two players in a family (and  $30.00  for each additional player in a family).  

Pre-K is $35.00 per player. Player must be 4 years old by August 31, 2008.

Important Information:   $10.00 late fee for registrations that are post marked after  July 18th   .

Picture Day:   August 23rd at Zearing Park.  If bad weather listen to WZOE to see if they have been moved to the

Prouty Building.  If they have been it will be the same time that you were scheduled.

Field Preparation Day:  Saturday, August 16th at 8:00 a.m.

Fall season will run from:  August 25th until October 11th.

I/We hereby agree that Princeton Youth Soccer League (PYSL), its members, coaches, officers, or those members, coaches, or officers of any organization to which PYSL is affiliated, shall not be liable for any injury or loss which my child or children may sustain while participating in activities of any kind, whether sponsored by, or under the supervision of PYSL or any other organization to which PYSL has an affiliation.  We further agree to indemnify and to hold harmless, PYSL, or any affiliated organization, its members, coaches, officers, or designates of any kind, from any claim what so ever.  I/We agree we are responsible for the return of borrowed equipment, including apparel, upon demand of PYSL.
